The GSA Office of Inspector General Office of Inspections conducted an inspection of a high-risk GSA building. The objective of the inspection was to determine whether GSA’s high-risk building’s security countermeasures were present and functioning according to the established guidance.
During the inspection, we conducted two site visits of the building, one announced and one unannounced. Our inspection determined that GSA did not ensure that all required security countermeasures were present and functioning. We found issues concerning obstructed and distorted security surveillance footage, unsecured vehicle access points, inadequate security guard stations, and unprotected non-window openings throughout the exterior of the building. In addition, during the unannounced inspection, we found unsecured and unattended GSA equipment and Controlled Unclassified Information.
Our inspection also found that GSA did not provide safety inspections or the required oversight of certain facility support operations. Required security controls and external coordination measures were not implemented, increasing the likelihood of GSA being unable to sustain specific facility support operations during an emergency or facility disruption.
The non-functioning and unimplemented facility security controls at the building create serious security vulnerabilities. Our report, issued on April 24, 2026, makes two recommendations for corrective action.
Our report addresses issues categorized as physical security information under GSA Order Security for Sensitive Building Information Related to Federal Buildings, Grounds, or Property, PBS 3490.3 CHGE 1, March 22, 2021. Therefore, our report requires safeguarding and dissemination controls and is not available to the public. The agency agreed with our report recommendations.
